Terms of Registration
In order to participate in the ATX 2025 Zebra Open, all participants, including coaches, managers and players, will be expected to complete an online registration by September 28, 2025, that is independent of any registration fees. During this process, each player will complete and electronically sign the Event Waiver & Release of Liability and Media Release, the contents of which can be seen below:
Terms for Team Sign-Up
In completing this registration on behalf of a team, I acknowledge that all members of the registered team, including coaches, managers and players, registered, must individually register online by September 28, 2025 (one week before the tournament) in order to participate in the ATX 2025 Zebra Open (“Event”). Player registration instructions will be provided.
I, as the team coach/manager/representative, agree to comply with all terms, including the requirement that each player/team member individually register online. During this process, each player will complete and electronically sign the Event Waiver & Release of Liability and Media Release. Central Texas Women’s Lacrosse Officials Organization’s (“CTWLOO”’s) intention is that the roster for each team will be automatically developed based on individual player registrations. No player will be permitted to participate without completion of the registration process.
CTWLOO relies on the responsibility of each team’s coach/manager/representative to ensure the eligibility of each player. CTWLOO is not responsible for teams and/or players who do not comply with this process, and any team that does not follow protocol is subject to forfeiture of any/all games.
Waiver & Release of Liability
Players WILL NOT be allowed to play without signed permission.
In consideration of my, or my child’s or ward’s participation in the ATX 2025 Zebra Open Girls’ Lacrosse Tournament (“Event”) hosted by Central Texas Women’s Lacrosse Officials Organization (“CTWLOO”), I agree to the following:
1. I am fully aware of and appreciate the risks, including the risk of catastrophic injury, paralysis and even death, as well as other damages and losses, associated with participation in a lacrosse event and related sports conditioning activities. I further agree on behalf of myself, my heirs and personal representatives, that CTWLOO, along with participating officials and coaches, shall not be liable for any injury, loss of life or other loss or damage occurring as a result of my participation in the Event.
2. I indemnify and hold harmless, CTWLOO, any individual working as an organizer, official, coach, athletic trainer, contractor or volunteer or in any capacity for this Event, for any and all injuries, damages, causes of actions or claims for personal injuries or property damage, arising from my, or my child's or ward’s participation in this event, including believed acts of negligence. I understand that my, or my child’s or ward’s participation includes possible exposure to COVID-19.
3. I hereby give my consent to CTWLOO to provide, through a medical staff of its choice, customary medical/athletic training attention, transportation and emergency medical services as warranted in the course of my participation in the Event. Such medical attention may be at my own cost, as warranted.
4. I declare that I, or my child or ward are physically fit and have the skill required to participate in this Event. I, or my child or ward, will only participate in those competitions or activities in which I believe I or they am/are physically and psychologically prepared to participate.
5. Assume all the foregoing risk and accept personal responsibility for the damages following such injury, permanent disability or death.
6. I also understand that my child or ward or I may be required to leave the Event and/or sports complex should my child or ward or I exhibit undesirable conduct.
7. I agree that I, or my child or ward, am/is a current USA Lacrosse member.
8. I agree to the refund policy.
Media Release
I grant Central Texas Women’s Lacrosse Officials Organization (CTWLOO), its subsidiaries, officials, officers, volunteers or contractors (collectively, “CTWLOO”), the right to photograph and/or videotape me or my child or ward at the ATX 2025 Zebra Open (“Event”) and further to display, use and/or otherwise exploit said photographs or video footage of my or my child’s or ward’s name, or my child’s likeness, image, photograph, voice, video, athletic performance, biographical, and other information (collectively, “Likeness”), in any media platform or format whatsoever, and to distribute, broadcast and exhibit these without charge, restriction or liability and such shall be the property of CTWLOO in perpetuity. I hereby release and discharge CTWLOO from any and all claims, demands, or causes of action arising out of or in connection with the use of my, or my child’s likeness for the purposes described herein.
